In a study published in 1999, the most common serotypes in children under 5 years in S. pneumoniae isolates were 6, 1, 19, 14, 4, 5, 45, 12 and 7.
In a study from Asian countries published in 2011, the serotypes detected were 19F , 23F , 6B , 1 , 5 , 7F , 19A , 15 .
In a recent study from three hospitals in Delhi published in 2013, out of 108 S. pneumoniae isolates tested, serotypes detected were – 19 , 6 , 7 , 1 , 14 , 9 , 33 , 17 , 11 , 3 , 18 , 23 , 12 , 32A , 15B , 22F , 5 , 29 , non-vaccine type E , F and H .
Although, the available data is patchy and is hospital based, the way forward is to begin with whatever data is available since community based surveillance to study the circulating serotypes representing the entire country will take a long time and monetary resources.
Based on data from the above three studies the most prevalent serotypes currently in India are 6A, 6B, 1, 19F, 19A, 14, 5, 7, 9V, 33 and 17.
As seen in , 13-valent vaccine covers two additional serotypes, i.e., 6A and 19A. Of these 19A alone is responsible for 13% of the vaccine type serotypes, whereas separate figures for 6A are not available. Thus, 13-valent PCV should be the vaccine of choice in India, provided its cost gets lower in line with PCV-10. This is evident from a recent study from India, which indicated that 54% of the pneumococcal isolates belonged to PCV-10 and 73% to PCV-13.

Although asymptomatic upper respiratory tract colonization is common, infection with S. pneumoniae may result in severe disease. IPD is a severe form of infection that occurs when S. pneumoniae invades normally sterile sites, such as the bloodstream or central nervous system. Bacteremia and meningitis are the most common manifestations of IPD in children 2 years of age and younger. Bacteremic pneumococcal pneumonia is the most common presentation among adults and is a common complication following influenza. The case fatality rate of bacteremic pneumococcal pneumonia is 5% to 7% and is higher among elderly persons. Bacterial spread within the respiratory tract may result in AOM, sinusitis or recurrent bronchitis.
Disease distribution
Worldwide, pneumococcal disease is a major cause of morbidity and mortality. The World Health Organization estimates that almost 500,000 deaths among children aged less than 5 years are attributable to pneumococcal disease each year. In Canada, IPD is most common among the very young and adults over 65 years of age.
Persons With Inadequate Immunization Records
Children and adults lacking adequate documentation of immunization should be considered unimmunized and should be started on an immunization schedule appropriate for their age and risk factors. Pneumococcal vaccines may be given, regardless of possible previous receipt of the vaccines, as adverse events associated with repeated immunization have not been demonstrated. Booster Doses Of Pneumococcal Vaccine
If you’re at increased risk of a pneumococcal infection, you’ll be given a single dose of the PPV vaccine.
But if your spleen does not work properly or you have a chronic kidney condition, you may need booster doses of PPV every 5 years.
This is because your levels of antibodies against the infection decrease over time.

Common And Local Adverse Events
Pneumococcal conjugate vaccine
Studies of Pneu-C-13 vaccine indicated that irritability decreased appetite increased or decreased sleep and pain, swelling and redness at the injection site after the toddler dose and in older children, are common side effects. Low grade fever occurred in 20% to 30% or more of vaccine recipients. In adults over 50 years of age, the most commonly reported side effects included pain at the injection site, fatigue, headache and new onset of myalgia, with fever above 38°C occurring in approximately 3% of vaccine recipients.
Pneumococcal polysaccharide vaccine
Reactions to Pneu-P-23 vaccine are usually mild. Soreness, redness and swelling at the injection site occur in 30% to 60% of vaccine recipients and more commonly follow SC administration than IM administration. Occasionally, low grade fever may occur. Re-immunization of healthy adults less than 2 years after the initial dose is associated with increased injection site and systemic reactions. Studies have suggested that re-vaccination after an interval of at least 4 years is not associated with an increased incidence of adverse side effects. However, severe injection site reactions, including reports of injection site cellulitis and peripheral edema in the injected extremity, have been documented rarely with Pneu-P-23 vaccine in post-marketing surveillance, even with the first dose. Multiple re-vaccinations are not recommended refer to Booster doses and re-immunization.

Medicare Coverage For The Pneumonia Shot
Part B of Original Medicare does cover the pneumonia vaccine, but there are certain limitations, according to Medicare.gov:
- Medicare Part B covers one shot. Anybody who is enrolled in Part B is entitled to a dose of pneumonia vaccine without having to pay for it if your health-care provider accepts Medicare assignment.
- Under certain circumstances, a doctor may prescribe a second shot of a different typeat least one year after the first dose. Part B may also cover this second dose.
- In either situation described above, you typically wont have out-of-pocket costs as a Part B beneficiary.
In some cases, a doctor may recommend more doses than the amount that Part B pays for. For example, a doctor may suggest a second dose of the PPSV23 vaccine. In this case, its possible that a Medicare Supplement or Medicare Advantage plan will provide coverage. Otherwise, you might have to pay for these additional services out of pocket. If the cost is a concern, its a good idea to contact Medicare or your Medicare plan to learn how these additional services will be covered or if they will be covered at all.

Geneva/ New York, 16 June 2020 Lower-income countries across the world will now be able to access life-saving pneumococcal conjugate vaccines, which protect against the leading cause of pneumonia, for US$ 2 per dose, thanks to a new supply agreement between UNICEF, Gavis procurement partner, and the Serum Institute of India . The new price represents a 43 per cent reduction from the Gavi price of US$3.50 at the start of the Advance Market Commitment .
The supply agreement is the eighth to take place under the Vaccine Alliances Advance Market Commitment mechanism, and the first to include a developing country manufacturer. Under the agreement, the Indian manufacturer will provide ten million PCV doses to Gavi-supported countries each year for the next ten years.The pneumococcus bacterium is the leading cause of severe pneumonia and is a major cause of morbidity and mortality worldwide. Most of these deaths occur in lower-income countries and include a disproportionate number of children under the age of two.
At last weeks Global Vaccine Summit, Gavi launched the Gavi Advance Market Commitment for COVID-19 Vaccines , a new financing instrument aimed at incentivising vaccine manufacturers to produce sufficient quantities of eventual COVID-19 vaccines, and to ensure access for developing countries.
