Get Proof Of Your Vaccination Status

Getting proof of vaccination status with My Covid Record | Ministry of Health NZ

Through the My Covid Record website you can now request two official records of your COVID-19 vaccination status.

  • My Vaccine Pass for use in Aotearoa New Zealand
  • International Travel Vaccination Certificate for overseas travel

You can also call or visit a participating vaccination site to request a pass or certificate.

To update NHI information, such as a name, call and select option 4.

Sign up for MyIR Mobile by going to myirmobile.com and follow the registration instructions. Your registration information will be used to match your records with the state immunization registry. You will be sent a verification code on your phone which will finalize the process.

Once registration is complete, you’ll be able to view your immunization records and access your COVID-19 vaccination certificate. Printed records from MyIR Mobile do not meet school immunization requirements, so please sign up for MyIR.com for valid school immunization documents.

Get Your Vaccine Certificate With Qr Code

Using your enhanced vaccine certificate with a QR code will make entering businesses and organizations more secure and convenient.

As of January 4, 2022, the enhanced vaccine certificate with official QR code will be required to access certain businesses and organizations. Vaccine receipts without a QR code will no longer be accepted as valid proof of vaccination.

  • not alter the appearance of your receipt. Vaccine certificates can be laminated but it may impact the readability of QR code
  • ensure that any information you provide is complete and accurate

REGISTER

Follow the registration steps by entering your personal information. Why are we asking for your personal information? For your safety and security we need to verify that the records you are requesting belong to you.

VERIFY

Your registration information is used to locate an exact record match in the immunization registry. If an exact match is found, a verification code will be sent to the matching phone number. You must enter that code, then your record will be linked and can be viewed on the Immunizations page.

If we are not able to find a match, you may be able to link your records by visiting a participating pharmacy or provider in your state.

ACCESS

You can now quickly access your familys immunization records and print from MyIR Mobile anytime, and anywhere.

Understanding The Differences Between Information Sources And Whats Been Uploaded

You can filter by source using the drop-down list in the Immunisation history section.

Some immunisation information may be duplicated, or some may be missing. This depends on where your healthcare provider uploaded your information.

  • Some information may appear twice if your healthcare provider uploaded it to both the AIR and your My Health Record .
  • Some information may be missing if your healthcare provider did not, or could not, upload it to either the AIR or your My Health Record.
  • Some information may appear in one source, but not the other. For example, your latest shared health summary might not include immunisations that have been recorded in the AIR.
  • Alerts for due and overdue immunisations relate only to immunisations recorded in AIR.

    From November 2018, the National Human Papillomavirus Vaccination Program Register transferred all HPV vaccination records to the AIR.

    It is the role of the vaccination provider to send details of your vaccinations to the AIR.

    What To Do If Your Immunisation Details Are Incomplete

    If the information in your Immunisations page is empty or seems incomplete:

  • Check your Medicare Information Settings .

  • Make sure Yes is selected for ‘All information about immunisations recorded on the Australian Immunisation Register .’

  • Selecting Yes will also let your healthcare providers see your immunisation information.

    Where Are Vaccinations Recorded

    Since October 2016 vaccinations in Australia have been recorded on the centralised Australian Immunisation Register . Prior to this, childhood vaccination records were kept on the Australian Childhood Immunisation Register .

    Earlier than 1996, records were kept locally with the vaccination provider such as the local council immunisation service or your GP. They may also have been recorded in a baby health book given to your parent or carer to keep at home.

    If You Need Another Dose Of Covid

    Bring your CDC COVID-19 Vaccination Record card with you to your appointment if you need another shot of COVID-19 vaccine so your provider can fill in the information about your additional shot.

    If you have lost your CDC COVID-19 Vaccination Record card or dont have a copy, contact your vaccination provider directly to access your vaccination record.

    • If you cannot contact your vaccination provider site directly, contact your state health departments immunization information system . Vaccination providers are required to report COVID-19 vaccinations to their IIS and related systems.
    • If you enrolled in v-safe or VaxText after your first vaccine dose, you can access your vaccination information using those tools.
    • If you have made every effort to locate your vaccination information, are unable to get a copy or replacement of your vaccination card, and need another COVID-19 vaccine dose, talk to a vaccination provider.

    CDC does not maintain vaccination records or determine how vaccination records are used, and CDC does not provide the white CDC-labeled COVID-19 Vaccination Record card to people. These cards are distributed to vaccination providers by state health departments.

    You do not need to keep your COVID-19 immunization record on you at all times however, you should print a copy of your record and store it in a safe place.

    If you received your record of immunization by email, you should print a copy and store it in a safe place.

    If you received a hard copy of your immunization record from the COVID-19 Immunization Clinic or partner pharmacy, you should store it in a safe place.

    Health Care Providers Records

    Health care providers must maintain a record of all vaccines that they administer and must ensure that information is accurately and completely recorded. In addition to recording information about the vaccines given , providers’ records should include:

    • all relevant serologic data, for example, rubella or hepatitis B serology)
    • documentation of adverse events following immunization
    • documentation of contraindications or reasons for deferring or withholding immunization
    • other immunization related documentation, such as pre-vaccine and provider administration check lists .

    Electronic medical records used by health care providers should have the capacity to record, to collect and to retrieve easily all information outlined in Immunization record contents, and should permit production of line listings of persons who received a specific vaccine in the event that the vaccine is recalled.

    • Unlicensed restaurants that don’t offer table service

    • Table service means providing any service to a customer at their seat, or from anywhere but behind a counter
  • Tasting rooms without seating attached to wineries, breweries or distilleries
  • Local public transportation
  • Salons, hairdressers and barbers
  • Hotels, resorts, cabins and campsites
  • Does not include settings or events covered by the PHO order, for example, a licensed hotel restaurant, wedding reception or conference
  • Does not include exercise/fitness facilities for guests
  • Banks and credit unions
