How To Get A New Vaccine Card

If You Have Lost Your Vaccination Record Card

Explained: How to Replace Your COVID-19 Vaccine Card

Your vaccination provider is usually the best source for your vaccine information. We encourage you to contact them directly.

Your vaccination information may be in the CVMS Vaccine Portal, if you received your vaccine or booster from a North Carolina pharmacy, grocery store, doctor’s office, hospital, health department, or community event:

This Is So Complicated

It is certainly easier in some places than others. Those C.D.C. cards are too big to fit in a wallet, making them unwieldy and, arguably, prone to being displaced. Thats one reason many states took a more digital approach.

In New York, for example, you can access the city Department of Healths website and obtain vaccination records, a spokesman for the department told me.

Getting records from the citys site, My Vaccine Record, is pretty fast. I punched in some biographical information and within seconds I got my vaccine history. It was three pages long, so I may not hang the whole thing on my fridge.

How Can I Store My Covid

The easiest method is to take a picture but not a selfie. If you upload a selfie with your vaccination card online, you could expose personal information that’s written on the card. Instead, on an iPhone, snap a picture of your card, go to the Share button on Photos, then select Hide. This will conceal your image in the Hidden album, which can be found by tapping Albums, then scrolling to Utilities.

For Google Pixel or Samsung Galaxy smartphone users, there’s an option to create a locked folder where you can store the picture of your COVID-19 vaccine card.

Depending on which state you live in, you can use newly designed apps to store your card information. Quick access is available for New Jersey and New York residents through the Docket and Excelsior app.

If you have an account with Walmart or Sam’s Club, you can take advantage of their offer to provide access to digital COVID-19 records. A COVID-19 vaccine digital passport can be added to Google Pay or Apple Wallet via VaxYes.

If you’d like to keep your vaccine card long-lasting, Staples customers can use the coupon code 81450 to get their vaccine card laminated. Staples hasn’t announced an expiration date on this offer yet.

However, you may want to think twice before laminating your vaccine card, in the case you need to document a booster COVID-19 vaccine dose. If you laminate your card, you can ask for another at your vaccination provider, AARP reports.

How To Replace Your Covid Vaccine Card If You Went To A Mass Vaccination Site

You can probably still replace your COVID-19 vaccine record, even if you got your shot at a church or stadium or a pop-up vaccination site that’s no longer around. Contact your county state’s health department: Most states have a phone number, email address and website where you can access your immunization records. You likely need to fill out an immunization request form and submit a photo ID. The process and how long it takes can vary by state.

Keep in mind that your state’s health department may not have access to your COVID-19 vaccine status if you were not vaccinated by a state-approved provider. It could also be possible that your vaccine record has not been updated on your state’s registry yet.

Your state may also have an online portal where you can retrieve your vaccine info, like North Carolina’s COVID-19 Vaccine Portal. North Carolinians that got vaccinated by one of the state’s providers and provided an email address can print their vaccine record from the portal.

Once You Get Your Records Save It

Vaccination cards will be issued to everyone getting Covid ...

Many places that require proof of vaccination will accept a picture of your Covid-19 vaccination card. So, once you get it, photograph it. Just make sure its in focus.

And if you received a QR code with your vaccination records, you can store it in the digital wallet on your iPhone . Those are harder to lose!

There are also apps from Clear, Common Pass, Docket, and IBM that can help you keep track of your vaccination record.

Could Your Doctor Give You Another Card

It depends. Your doctor might have extra C.D.C.-issued white cards. But they should at least have a record of when and where you received your vaccine shot and booster. Ask them.

And most states require health care providers who administer vaccine shots to log that information with state health officials. So, state health officials should have that info too.

If I Was Vaccinated In Another State And Lost My Card How Can I Get Another One

If you received one or both of your doses in another state, you will need to contact that states Immunization Information System, and have them provide you with a copy of your vaccination details. You can then provide that to your doctor or parish health unit, and have them enter your information into LINKS.

Once your information has been entered in LINKS, it will be visible to you on the MyIR website.

You can register or log into MyIR to see all of your Louisiana vaccination records, and print out a copy of your records, which will also serve as proof of vaccination status. You can access MyIR here.

With the new Omicron coronavirus variant spreading like wildfire, businesses like bars and restaurants are likely to require proof of vaccination before letting folks in through their doors .

While the Centers for Disease Control and Preventions COVID-19 vaccination cards are simple to read, their four-by-three inch size makes them awkward to carry around because standard wallets are 2.5-by-3.5 inches. Plus, the card’s simplicity makes it easy to counterfeit because it consists of regular black ink printed on plain card stock.

    Check With Your Doctor’s Office

Did you get vaccinated at a doctor’s office, clinic or health care facility? If so, they may have a record of your shot. If not, they’ll be able to point you in the right direction.

Your doctor’s office or clinic may use an app, like MyChart, that stores your proof of vaccination. Some physicians put vaccine documents and test results in the app, so it’s easy to grab. But keep in mind that even if your information is available from the app, you may not be able to get a replacement card with the CDC’s seal if your doctor’s office doesn’t offer one. However, you may be able to ask for a printed version of your vaccine record.

I Lost My Vaccine Card How Can I Get A New One

People who have lost their vaccine cards before getting their second dose can ask for a new card at their second vaccine appointment. If you need to do that, make sure that the first dose is also recorded on that new card.

The Minnesota Department of Health does not have replacement cards for those who have lost their COVID-19 vaccine card or for those who have ended up with two vaccine cards. However, you can review your vaccine history through the Minnesota Department of Healths Find My Immunization Record.

Last week, the state launched a new app called Docket allowing people to view and access their immunization records including for COVID-19 instantaneously. Users will be able to save a PDF document of their records.

Health care providers, pharmacists and other practitioners submit vaccination information to the state database called the Minnesota Immunization Information Connection every time they administer a shot.

The Docket app gives Minnesotans a digital option to access their immunization history in MIIC, check what vaccines you or your children may be due for, and see what vaccines you may need in the future, said state infectious disease director Kris Ehresmann. This is vital to making sure people are protected from preventable diseases.

    About Your Vaccine Card

    • If you need an additional dose of the COVID vaccine or a booster shot, bring your COVID-19 vaccination card with you to your appointment so the information can be recorded.
    • Do not carry your vaccine card with you on a daily basis. Instead, keep your vaccine card in a secure location. Use your smartphone to take a picture of your card so that you always have a record of it with you.
    • It is not recommended to laminate your vaccination card. If your vaccine information is recorded on the card using a printed label, laminating the card could burn the label, making it impossible to read. Also, as the research on COVID immunity becomes clearer, you may require a booster shot in the future, which will need to be recorded on the card. The best way to protect your vaccine card is by keeping it in a secure location. The CDC also recommends that you make a backup copy by taking a photo of the card.
    • Protect yourself from scammers. There has been a lot of misinformation spread about COVID-19 and the vaccines, leaving the door open for scammers to take advantage of vulnerable people. Be aware when receiving any unexpected phone calls, e-mails or text messages. Do not click on links from unknown senders, answer questions about your personal medical history or provide financial information.
