If You Need Another Dose Of Covid
Bring your CDC COVID-19 Vaccination Record card with you to your appointment if you need another shot of COVID-19 vaccine so your provider can fill in the information about your additional shot.
If you have lost your CDC COVID-19 Vaccination Record card or dont have a copy, contact your vaccination provider directly to access your vaccination record.
- If you cannot contact your vaccination provider site directly, contact your state health departments immunization information system . Vaccination providers are required to report COVID-19 vaccinations to their IIS and related systems.
- If you enrolled in v-safe or VaxText after your first vaccine dose, you can access your vaccination information using those tools.
- If you have made every effort to locate your vaccination information, are unable to get a copy or replacement of your vaccination card, and need another COVID-19 vaccine dose, talk to a vaccination provider.
CDC does not maintain vaccination records or determine how vaccination records are used, and CDC does not provide the white CDC-labeled COVID-19 Vaccination Record card to people. These cards are distributed to vaccination providers by state health departments.

Health Care Providers Records
Health care providers must maintain a record of all vaccines that they administer and must ensure that information is accurately and completely recorded. In addition to recording information about the vaccines given , providers’ records should include:
- all relevant serologic data, for example, rubella or hepatitis B serology)
- documentation of adverse events following immunization
- documentation of contraindications or reasons for deferring or withholding immunization
- other immunization related documentation, such as pre-vaccine and provider administration check lists .

The Department of Health uses the Washington State Immunization Information System, but it does not have complete immunization records for all people. When you call, you’ll be asked to provide identifying information, such as name, date of birth, and address. At that time, we will tell you whether or not a record exists in the system, but no other information will be released over the phone. This is to protect the privacy and confidentiality of your information.

And all of the European Unions 27 member countries are now using the EU Digital Covid Certificate, which allows for free movement between all the countries within the bloc.
But digital verification is becoming more commonplace at home, too. Anyone road-tripping across the United States will likely encounter more than one restaurant, entertainment venue or sports stadium that requires proof of vaccination as a condition for entry.
The Biden administration has declined to get behind a national vaccination verification app here in the United States, and the topic of vaccine passports is a political hot potato, with 15 Republican-leaning states limiting or banning vaccine passports via legislation or governors executive orders.

A private-public partnership of technology and health care companies called the Vaccination Credential Initiative , which includes Microsoft, Salesforce, Oracle, the Mayo Clinic and other heavyweights, has created a verifiable digital proof of vaccination called the SMART Health Card that has been gathering steam.

Experts stress that COVID-19 vaccines are a vital part of ending the pandemic, and even if their effectiveness wanes, they provide key and important protection against severe disease.
One of the misunderstandings that people have about the COVID vaccine, and theres lots of reasons why they are confused they think that if youre vaccinated, you wont get the virus, said Peter Pitts, Former FDA Associate Commissioner and president of the Center for Medicine in the Public Interest.
While thats true for many people, he explained, the vaccine doesnt provide complete protection from infection.
Especially when we have a mutation thats highly infectious, Pitts said. It mitigates the symptoms.
He added that with currently available vaccines, the mitigation of serious symptoms and hospitalizations is extremely robust, as witnessed by high infection, low hospitalization.
The reality is that the vaccines as they currently exist are extremely effective at accomplishing their mission of keeping people out of the hospital and dying, confirmed Pitts.
Pitts said it is expected that vaccine efficacy will drop over time. This may partly be due to waning antibodies, but this study did not measure antibody levels.
Pitts explained that this research proved a hypothesis that vaccine protection wanes over time, and the vaccine provides greater protection against some variants than others.
But its still effective, he said. It isnt necessarily less effective.
