Why Does The Credential Only Have 1 Page
As of November 18, 2021, the Yukon credential now only has 1 page. The credential used to have 3 pages. We changed this to make it easier for Yukoners to use the credential. You do not need to re-download your credential if you have the 3 page version as it is still valid. The credential contains both a QR code and the information listed below:
- Your full name.
- The vaccine product name .
- The date you got your vaccinations.
- The vaccine lot number.

You can probably still replace your COVID-19 vaccine record, even if you got your shot at a church or stadium or a pop-up vaccination site thats no longer around.
Contact your county states health department: Most states have a phone number, email address and website where you can access your immunization records. You likely need to fill out an immunization request form and submit a photo ID. The process and how long it takes can vary by state.
Keep in mind that your states health department may not have access to your COVID-19 vaccine status if you were not vaccinated by a state-approved provider. It could also be possible that your vaccine record has not been updated on your states registry yet.
Your state may also have an online portal where you can retrieve your vaccine info, like North Carolinas COVID-19 Vaccine Portal. North Carolinians that got vaccinated by one of the states providers and provided an email address can print their vaccine record from the portal.

Federal Proof Of Vaccination For Travellers
The new Canadian COVID-19 proof of vaccination document, which includes a QR code, is now available on MyHealthNB.
This document uses the federal standard format adopted across Canada, which is accepted by Canada Border Services Agency and ArriveCAN. It provides New Brunswickers who are travelling to jurisdictions that are using QR codes with a reliable way to show they are vaccinated against COVID-19.
It is recommended that people travelling internationally verify the travel requirements for their destination well in advance. Each country has its own regulations about what type of vaccination proof is required, the types of vaccines accepted, the number of doses required and any exemptions to public health measures.
When using the Canadian COVID-19 proof of vaccination, it is recommended that you download the full PDF document, save the QR code as a JPG image file to your device and print a paper copy. This will ensure your destination is able to validate that your vaccination requirements have been met, regardless of whether they use a verifier application to scan the document.
Within New Brunswick, the original paper immunization record, a photograph or digital copy of the immunization record or the digital vaccination status on MyHealthNB will continue to be used for proof of vaccination, along with a valid government ID.

The GNWT is now issuing electronic Proof of Vaccination Credentials to individuals aged 5 and older.
A self-service online form allows you to download your digital proof of vaccination.
You can use this government-issued credential to prove your COVID-19 vaccination status where required.
You will require a new credential,
- If you have received additional doses of vaccination since you received your Proof of Vaccination Credential or
- If the format or period of validity of the credential changes due to evolving national or international standards.

Where You Will Need Proof Of Vaccination
Proof of vaccination allows people age 12 and up who are adequately protected or who have a clinical contraindication to the COVID-19 vaccines available to be admitted to certain locations and certain non-essential activities where the risk of transmission is high. These include:
- Indoor festivals, performing arts and sporting events
- Indoor and outdoor dining and drinking at restaurants, pubs and bars
- Nightclubs, amusement centres, pool halls, bowling alleys and casinos
- Movie theatres, museums and libraries
- Gyms, indoor group exercise, indoor pools and recreation facilities
- Indoor organized gatherings like weddings, funerals, parties , conferences and workshops

You will need to show government issued identification along with your proof of vaccination. i.e. driverâs license, Medicare card, birth certificate, government ID, etc.
A previous COVID-19 infection or a negative COVID-19 test result can not be used as proof of vaccination.

Proof of vaccination is also required for some on-campus housing.
