The records that exist are the ones you or your parents were given when the vaccines were administered and the ones in the medical record of the doctor or clinic where the vaccines were given.

The Michigan Care Improvement Registry is an immunization database that documents immunizations given to Michiganders throughout life. The Michigan Care Improvement Registry was created in 1998 to collect reliable immunization information for children and make it accessible to authorized users. An Official TN Immunization Certificate must be submitted to childcare centers, preschools, or schools to satisfy attendance requirements for childcare through 12th grade. This certificate is available at your local TN Health Department and at any medical office with TennIIS access.
