Cdc Numbers Don’t Include Many States
The federal government collects information about vaccine waste through federal systems called VTrckS, which manages ordering and shipments, and Tiberius, a platform run by the Department of Health and Human Services that monitors distribution. VTrckS can exchange data with state and local immunization registries that track who has received shots, but some states rely on manual data entry, Hannan said.
The 15 states not included in the CDC’s data are Alaska, California, Colorado, Kansas, Louisiana, Maine, Maryland, Michigan, Nebraska, Nevada, New Hampshire, Ohio, Oklahoma, Oregon and Texas. The District of Columbia is also missing.
Of those jurisdictions, 11 provided data to KHN: Alaska, Colorado, Kansas, Maryland, Nebraska, Nevada, New Hampshire, Ohio, Oregon, Texas and the District of Columbia.
Most of those reported minimal waste to KHN: Colorado, Kansas, Nebraska and the District of Columbia together registered just 1,090 wasted doses.
In others, the numbers are more significant. On March 19, the Maryland Health Department said it knew of 3,175 wasted doses.
Texas had the most wasted doses of any state in either the CDC’s data or the data states provided to KHN. Its records showed 9,229 wasted doses as of March 26, third overall behind CVS and Walgreens.
Fowlie, the CDC spokesperson, said the agency is “working closely” with states that have technical issues to ensure accurate reporting.

Vaccine administration surged more than 30% in states such as Alabama, Florida, Georgia and Kentucky in the past few weeks, the drugstore chain said.
The company, among the largest U.S. pharmacy chains, had said in June that it expects to administer 28 million vaccines this year.
Walgreens also said it had made masks mandatory for its U.S. employees and required staff working at U.S. support offices to be fully vaccinated, the latest company to update its policies as Covid-19 cases rise in the United States.
New guidance from the U.S. Centers for Disease Control and Prevention that recommends fully vaccinated individuals wear mask as well as surging cases due to the more infectious delta variant have led companies to rethink their policies.
Walgreens said on Wednesday it also encouraged all customers, including those who are vaccinated, to wear masks.

Individuals eligible to receive the additional vaccine dose include people undergoing cancer treatment, stem cell or organ transplant recipients, people living HIV or those receiving immunosuppressive treatments, according to the release.
The extra shots were recommended for Pfizer recipients who are 12 or older and Moderna recipients who are 18 and older, CNBC reported. The Advisory Committee on Immunization Practices said it would revisit the shots for younger Moderna recipients after the FDA clears the shots for kids.
Immunocompromised patients make up roughly 2.7% of the U.S. adult population and 44% of hospitalized breakthrough infections, where someone gets infected even after they’ve been fully vaccinated.

The reasons states gave for waste varied, from broken vials and syringes to providers’ storage errors to leftover doses from open vials that couldn’t be used.
The largest waste incidents, in which hundreds of doses were lost at a time, tended to be due to freezer malfunctions or workers’ leaving doses at room temperature too long.
But state records also register the little things that can go wrong.
On Dec. 16, the Public Health Department in Gunnison County, Colorado, lost a single dose of the Pfizer vaccine when someone bumped into a table and a vial spilled. On Jan. 5, the Tri-County Health Department in Westminster, Colorado, reported that it wasted a Moderna dose because a hypodermic needle bent.
Remi Graber, a registered nurse who has vaccinated people at mass sites and community health clinics in Rhode Island, said it’s not uncommon for a vial to have one too many or one too few doses, which can lead to a dose’s being counted as wasted. Syringe problems also sometimes result in waste.
But Graber said the biggest problem is people who don’t show up. Once a vial is punctured, Pfizer’s vaccine must be used within six hours. On April 1, Moderna announced that an opened vaccine vial was good for 12 hours â double what it had been previously.

Cvs Walgreens Have Wasted More Covid Vaccine Doses Than Most States Combined
Two national pharmacy chains that the federal government entrusted to inoculate people against Covid-19 account for the lion’s share of wasted vaccine doses, according to government data obtained by Kaiser Health News.
The Centers for Disease Control and Prevention recorded 182,874 wasted doses as of late March, three months into the country’s effort to vaccinate the masses against the coronavirus. CVS was responsible for nearly half, and Walgreens was responsible for 21 percent, or nearly 128,500 wasted shots combined.
CDC data suggest that the companies have wasted more doses than states, U.S. territories and federal agencies combined. Pfizer’s vaccine, which in December was the first to be deployed and initially required storage at ultracold temperatures, made up nearly 60 percent of the tossed doses.

It’s not completely clear from the CDC data why the two chains wasted so much more vaccine than states and federal agencies. Some critics have pointed to poor planning early in the rollout, when the Trump administration leaned heavily on CVS and Walgreens to vaccinate residents and staff members of long-term care facilities. In response to questions, CVS said “nearly all” of its reported vaccine waste occurred during that effort. Walgreens didn’t specify how many doses were wasted during the long-term care program.

Walgreens Administered Covid Vaccines Wrong Now Its Trying To Follow The Rules

Walgreens has come under fire for incorrectly administering second doses of the Pfizer COVID vaccine.
According to a report from The New York Times, the pharmacy and drug store retailer has been giving patients the second dose of the Pfizer vaccine four weeks after the first because it was easier to schedule at the same interval as the Moderna vaccine.
The second dose of the Pfizer COVID vaccine needs to be provided in a three-week interval, while the second dose of the Moderna COVID shot is administered at a four-week interval.
Dr. Kevin Ban, Walgreens’ chief medical officer, told The Times that administering the second dose of the Pfizer vaccine in the fourth week was the easiest way to stand up the process based on our capabilities at the time.
The Centers for Disease Control and Prevention has acknowledged the error, requesting that Walgreens administer the vaccine doses correctly, the Times reported. However, the CDC does say in a guideline on its website that, in some instances, it is acceptable to extend a second dose of the Pfizer vaccine up to six weeks if necessary.
Kate Grusich, a CDC spokeswoman, told USA Today in an email, For individuals who received the Pfizer COVID-19 vaccine, CDC continues to recommend getting the second shot three weeks after the first shot, and to stick as close as possible to that recommended three-week interval.

An Indiana couple said a Walgreens pharmacy gave their kids a COVID-19 vaccine instead of flu shots.

“Walgreens called me to say there was a mix up, we did not receive the flu shot,” Alexandra Price told WFIE.

A similar incident happened in Baltimore, Maryland, where a 4-year-old received the Pfizer vaccine.
A couple in Indiana claim a Walgreens pharmacy mistakenly gave their 4- and 5-year-old children adult doses of the COVID-19 vaccine instead of flu shots, local news outlet WFIE reported.
Alexandra and Joshua Price said they took their children to Walgreens on October 4 to get their flu shots, but instead they received doses of the Pfizer COVID-19 vaccine.
The Pfizer vaccine is authorized for use in children 12 and older and is seeking approval by the Food and Drug Administration for children ages 5 to 11. They have not requested eligibility for children under the age of five.
“Walgreens called me to say there was a mix-up, we did not receive the flu shot,” Alexandra Price told WFIE. “And I’m like well what did we get? And he was like we got the COVID-19 shot, And instantly I was like, well what does this mean for my kids?”
Daniel Tuley, who represents the Price family, showed WFIE the vaccination cards for their 5-year-old daughter and 4-year-old son, showing that they were born in 2016 and 2017, respectively.
Tuley did not immediately return Insider’s request for comment.
