Immunization records that exist for adults are usually records that were provided to their parents when they received vaccines as children and in the adults medical record of the physician or clinic where vaccines were given. There is no national database that maintains the immunization records for adults.
If official copies of immunization records are needed or if there is a desire to update personal records, the following places may provide some assistance to obtaining these records:
- Request immunization records from parents or caregivers if they still have them.
- Search baby books or other saved documents from childhood
- Contact the high school and/or college health services attended for dates of any immunizations received. Please be advised that records are only maintained for a certain length of time.
- Contact previous employers that may have required immunizations for employment purposes.
- Contact your health care provider or county health department. Please be advised that records are maintained by the health care providers office for a limited number of years.
- Check with the county health department to determine if you have immunizations recorded in the Florida SHOTS system.

Many health departments can provide you with your vaccination information. The records themselves arent lost.
But if youre hoping to use that card to prove your vaccination status at work or, say, to travel to one of those countries with a more digital-first approach to record keeping, losing it may make it harder for you to take part in those markers of everyday life like being around other people. - If you were vaccinated in Alberta but don’t have an Alberta Personal Health Number, call 811 to get a vaccine record with QR code mailed to you.
- If you become eligible for an Alberta Personal Health Number, you can use it to access your vaccine record online at alberta.ca/CovidRecords.
Alberta residents vaccinated outside Alberta
- If you or your children under 18 were partially or fully vaccinated outside of Alberta, update your health records by submitting your digital out-of-province/country immunization records online at ahs.ca/vaccineregistry.
- Submitted records will be reviewed by AHS:
- If more information is required to validate your record, AHS will contact you.
- If your record cannot be validated, a letter will be sent. Book an appointment to begin or complete a valid COVID-19 vaccination series.
New to Alberta
- If you recently moved to Alberta, use your provincial vaccine record with a QR code or out-of-country vaccine record.
- New residents who were not vaccinated in Alberta cannot get an Alberta-issued vaccine record.

But as the coronavirus pandemic rolled on, and the bandages came off, some people simply lost track of those awkward 4.25-by-3.5-inch white cards.

Check With Your Doctor’s Office
Did you get vaccinated at a doctor’s office, clinic or healthcare facility? If so, they may have a record of your shot. If not, they’ll be able to point you in the right direction.
Your doctor’s office or clinic may use an app, like MyChart, that stores your proof of vaccination. Some physicians put vaccine documents and test results in the app, so it’s easy to grab. But keep in mind that just because your information is available on the app, you may not be able to get a replacement card with the CDC’s seal if your doctor’s office doesn’t offer one. However, you may be able to ask for a printed version of your vaccine record.
Finding Official Vaccination Records
If you dont have a copy of your childs vaccine records or cant find them, you may be able to retrieve an official copy by contacting your:
- Childs doctor or clinic
- Doctors and public health clinics usually track any shots they give to your child.
- If your child has had more than one doctor or clinic give him or her shots, call or visit each one to get the records.
- Keep in mind doctors and clinics may only save vaccination records for a few years.

What To Do If Your Immunisation Details Are Incomplete
If the information in your Immunisations page is empty or seems incomplete:
Check your Medicare Information Settings .
Make sure Yes is selected for ‘All information about immunisations recorded on the Australian Immunisation Register .’
Selecting Yes will also let your healthcare providers see your immunisation information.

I Was Vaccinated At Cvs Or Walmart Or Walgreens Can They Replace My Card
Youre in luck.
CVS: A record of the vaccination is available to patients via the CVS Pharmacy app, or on the companys website, a spokesman said. A pharmacy employee can print a paper record for you, the spokesman said.
Walgreens: The company keeps records of all vaccinations administered by its pharmacies, according to a spokeswoman. If patients lose their physical card, they are encouraged to contact their pharmacy for a new one, the spokeswoman said.
Walmart: If you were vaccinated at a store or one of its sponsored events, Walmart can verify your information and connect with your states immunization registry. Once that is complete, Walmart will reissue a vaccine card to reflect the doses administered at Walmart, a spokeswoman said in a statement. The company can also provide that information digitally, or via a QR code.

Yes, once your vaccine document is approved and added to your personal PEI immunization file by program staff you will be able to access your record online through the provinces COVID-19 Immunization Record portal. Allow 10 business days, or longer depending on volume of requests, for your vaccine information to be added to your personal PEI immunization record.
Issues With Your Vaccine Record
Contact a vaccine provider if:
- You have a valid MCP number and received a COVID-19 vaccine in Newfoundland and Labrador, but get the message Your COVID-19 vaccine record could not be found.
- There are errors in your vaccine record.
- You have questions or concerns about the content of your vaccine record.

How Can I Send Immunization Data To The Idph
The IDPH has implemented HL7 data exchange using two methods. The first method is one-way data transfer of unsolicited vaccine record updates to the statewide registry. This method accepts batch immunization data using the Departments Secure File Transfer Protocol system or the I-CARE application.
The second method is a real-time, bidirectional data exchange of vaccine record updates, queries, and responses. This method allows immunization providers using their EHR system to query I-CARE for a patient and ICARE will send the patient immunization information back. It also will allow providers to send updates from their EHR directly to I-CARE for immediate update. This method uses the Hypertext Transfer Protocol Secure with web services.
Could Your Doctor Give You Another Card
It depends. Your doctor might have extra C.D.C.-issued white cards. But they should at least have a record of when and where you received your vaccine shot and booster. Ask them.
And most states require health care providers who administer vaccine shots to log that information with state health officials. So, state health officials should have that info too.

- If you enrolled in v-safe or VaxText after your first vaccine dose, you can access your vaccination information using those tools.
- If you have made every effort to locate your vaccination information, are unable to get a copy or replacement of your vaccination card, and need another COVID-19 vaccine dose, talk to a vaccination provider.
CDC does not maintain vaccination records or determine how vaccination records are used, and CDC does not provide the white CDC-labeled COVID-19 Vaccination Record card to people. These cards are distributed to vaccination providers by state health departments.

Ontario’s chief medical officer of health shared some tips for residents looking to get their COVID-19 vaccination record in case you ever need proof.
While Doug Ford has explicitly said that Ontario will not roll out any kind of vaccine passport for residents, Dr. Kieran Moore noted there is an easy way to get your vaccination receipt, if you want it.
“Anyone that wants their certificates of vaccination, you can download it to get your two-dose documentation. You can use that as validation that you have been vaccinated, if that is ever required,” said Moore during Tuesday’s press conference.
“That means of validating has always been present and it’s downloadable through the Ontario.ca website. I’ve done the same and printed them out to have them if I’m ever asked, either in a workplace and/or if I’m travelling,” he added.

What If You Cant Find Your Childs Records
Your child should be considered susceptible to disease and should be vaccinated if you cant find his or her records or their records are incomplete. It is safe for your child to receive a vaccine, even if he or she may have already received it. Alternatively, your child could also have their blood tested for antibodies to determine his or her immunity to certain diseases. However, these tests may not always be accurate and doctors may prefer to revaccinate your child for best protection. Talk to your childs doctor to determine what vaccines your child needs for protection against vaccine-preventable diseases.

Submitting And Exchanging Data
If you are a provider and use an Electronic Health Record system, you may be able to share data with I-CARE using HL7 data exchange. You will need to contact your EHR vendor to determine if your EHR system is HL7 compatible. You also may need to acquire an additional interface for your EHR to send and exchange immunization data.

Once You Get Your Records Save It
Many places that require proof of vaccination will accept a picture of your Covid-19 vaccination card. So, once you get it, photograph it. Just make sure its in focus.
And if you received a QR code with your vaccination records, you can store it in the digital wallet on your iPhone . Those are harder to lose!
There are also apps from Clear, Common Pass, Docket, and IBM that can help you keep track of your vaccination record.
Boost Protection With A Third Dose
Vaccine effectiveness against infection may decline over time especially with the highly transmissible Omicron variant but they remain the best way to prevent severe illness and death. Third doses can boost immunity to improve protection and limit spread.
Everyone 18 and older can get a third dose 5 months after their second dose.
