Who Should Not Receive Typhoid Vaccines

Very few people cannot be given the injectable typhoid vaccine. It should not be given to:

  • People with an illness with high temperature . It is best to wait until the illness has subsided before being immunised.
  • People who have had a severe reaction to the vaccine in the past.
  • Young children – when children are too young to be vaccinated, very careful attention to food and water hygiene is essential if taking children to an at-risk area.

There are currently no data on the safety of these vaccines in pregnant or breast-feeding women. However, if the risk of typhoid is high then you may be advised to have the vaccine if you are breast-feeding or pregnant.

There are some additional restrictions that apply differently to the oral vaccine and to the injectable vaccine.

The oral vaccine should not be given to:

  • People who have had an allergy to gelatin.
  • Children under the age of 6 years.
  • People who have reduced immunity .

The injectable vaccine should not be given to:

  • Children under 2 years of age, as it is not licensed for them. This means that the manufacturer cannot offer guarantees about its effects or effectiveness in that age group.
  • Children under the age of 2 years, who may not acquire the same level of immunity from the vaccine, due to the immaturity of their immune systems.

Note: doctors may occasionally recommend injectable vaccine use in children aged 12-24 months if the risk of typhoid is very high.

Measles Mumps And Rubella Vaccination

WHO recommends use of first typhoid conjugate vaccine

The MMR vaccine that protects against measles, mumps and rubella is routinely given to all children in the UK.

You should make sure you and your children are up-to-date with routine vaccinations, including MMR, before travelling.

If you haven’t been fully vaccinated against these conditions or you’re not already immune, you should ask about MMR vaccination before you travel.

The MMR vaccine is given as 2 injections. These are usually given when a child is 12 to 13 months old and when they start school.

But if vaccination has been missed previously, adults can have the doses 1 month apart, and children can have them 3 months apart if necessary.

You should ideally have the final dose at least 2 weeks before you leave.

Oral Live Attenuated Vaccines

The attenuated non-pathogenic Salmonella Typhi strain Ty21a was derived by chemically attenuating a wild-type strain.

The Salmonella Typhi strain Ty21a cannot be detected in faeces more than 3 days after receiving the vaccine.

The vaccine stimulates: 1

  • vigorous secretory intestinal IgA
  • cell-mediated immune responses

Clinical trials have been done in several countries with endemic typhoid fever, including Chile, Egypt and Indonesia. The trials used different formulations of the vaccine and a variety of schedules. They revealed varying levels of protection against typhoid fever.1,9

A large clinical trial in more than 200,000 school children compared the efficacy of 2, 3 and 4 doses of enteric-coated oral typhoid vaccine.2 The study showed that 4 doses of the oral typhoid vaccine resulted in a significantly lower typhoid fever incidence than 3 doses.

Who Should Get The Typhoid Vaccine

The typhoid vaccine is not required for international travel. And the vaccine is not generally recommended for people traveling to areas where the infection is common. However, the typhoid vaccine is recommended for people who travel to high-risk areas if they plan on any of the following:

  • Travel “off the beaten path” in small towns and rural areas.
  • Travel for more than six weeks in countries where typhoid is common.
  • Travel and eat in areas without standard tourist accommodations.

Keep Away From Animals

Most animals avoid people, but they may attack if they feel threatened, are protecting their young or territory, or if they are injured or ill. Animal bites and scratches can lead to serious diseases such as rabies.

Follow these tips to protect yourself:

  • Do not touch or feed any animals you do not know.
  • Do not allow animals to lick open wounds, and do not get animal saliva in your eyes or mouth.
  • Avoid rodents and their urine and feces.

Where Does Typhoid Fever Occur

Destination of travel is the strongest predictor of typhoid risk. The highest risk area is South Asia, where the Public Health Agency of Canada estimates one in 3,000 travellers may develop typhoid fever. There is an intermediate risk for travellers to sub-Saharan Africa, North Africa, the Middle East, and South America.

The PHAC defines South Asia as Afghanistan, Bangladesh, Bhutan, India, Nepal, Maldives, Pakistan, and Sri Lanka. Among those countries, the majority of typhoid cases were reported in India, Pakistan, and Bangladesh.

    Doctors Support The Change

    Richard Watkins, MD, an infectious disease physician and a professor of internal medicine at the Northeast Ohio Medical University, tells Verywell that there was never any compelling evidence for the previous recommendation, adding, I am glad it has been changed.

    Watkins says that the move may help more children get vaccinated, noting the convenience factor. Under the updated guidance, families only have to make one trip to get vaccinated instead of several under the previous recommendations, he says.

    John Schreiber, MD, a pediatric infectious disease specialist at Connecticut Children’s Medical Center, tells Verywell that the changed guidance seems like a reasonable thing to do.

    Schreiber anticipates that some parents may still be wary to give their children other vaccines at the same time as the COVID-19 vaccine, but say that new recommendations are sound.

    I dont have any concerns with this, Schreiber says. But, he adds, the CDC and AAP will monitor children to see what happens next. If it turns out that children are complaining about more side effects after getting vaccinated, Im sure the recommendations can be modified.”

    PETALING JAYA, April 5 Malaysian doctors are calling on food handlers to ensure they are vaccinated against typhoid before the Ramadan month kicks off.

    Early prevention is necessary to prevent an outbreak from occurring especially amongst first-time business owners who have turned to home-based cooking or baking to make a living during the Covid-19 pandemic.

    In light of this, the Academy of Occupational and Environmental Medicine Malaysia has partnered with the Facebook group Caring Moms to launch a health education campaign titled Healthy Food Handlers, Food Safety Assured.

    The campaign will urge homegrown food handlers to practice good hygiene when preparing and delivering food and to maintain public confidence in the safety of their products by getting vaccinated against typhoid.

    What is typhoid?

    Typhoid, also known as typhoid fever, is a systemic infection that spreads through food and drinks contaminated with the Salmonella Typhi bacteria via oral-faecal transmission.

    Environmental factors that facilitate the spread of typhoid include poor sanitation, bad hygiene practices, and a lack of access to clean drinking water.

    Symptoms include fever, fatigue, headache, nausea, abdominal pain, constipation, diarrhoea and rashes, though 1 to 4 per cent of patients infected with S. Typhi are chronic, asymptomatic carriers.
